Choices provides a wide range of sexual health care services with compassion, and without judgment or discrimination. Our goal is to provide you with all the information and resources you need to protect your health and make informed decisions. We will always respect your sexual health decisions and your need for privacy and confidentiality.

NOTE: Prices shown on the website are discounted cash prices.  We are currently an in-network provider with the following plans:  Blue Cross Blue Shield, United Healthcare, Medicare, TennCare, and TriCare.  We can provide an itemized receipt for services if you would like to file an out-of-network claim with another plan. Please call your insurance carrier for details about your own coverage and benefits.

Wellness Exams for Men and Women    $75.00 - $155.00

Gardasil®, HPV Vaccine for Teens, Men, and Women

GARDASIL is a vaccine indicated in girls and women 9 through 26 years of age for the prevention of cervical, vulvar, and vaginal cancers; precancerous or dysplastic lesions; and genital warts caused by human papillomavirus (HPV) Types 6, 11, 16, and 18. GARDASIL is also indicated in boys and men 9 through 26 years of age for the prevention of genital warts caused by HPV Types 6 and 11.     Birth Control Services

    Birth Control Consultation      $55.00

    Includes a discussion of your medical history and birth control options with our nurse practitioner and a prescription for your method of choice. This type of appointment does not include lab work or a physical exam.

    ParaGard® IUD & Insertion  Visit fee + $655.00   NOW AVAILABLE AT NO COST TO YOU!
    This fee includes the copper Paragard® IUD and 2 appointments

    with our medical practitioner. Your first appointment will be a consultation and evaluation, and the IUD will be inserted during the second appointment.

    Mirena® IUS and Insertion   Visit fee + $730.00    NOW AVAILABLE AT NO COST TO YOU!

    This fee includes the Mirena® IUS and 2 appointments with our medical practitioner. Your first appointment will be a consultation and evaluation, and the IUS will be inserted during the second appointment.

    Implanon® and Insertion   Visit fee + $705.00        NOW AVAILABLE AT NO COST TO YOU!

    This fee includes Implanon® and 2 appointments with our medical practitioner. Your first appointment will be a consultation and evaluation, and the Implanon will be inserted during the second appointment by our Implanon® Accredited Provider.

    Depo Provera Shot  $35.00 

    This fee is for established patients, and it includes the Depo Provera shot and a consultation with our medical practitioner. If you are a new patient, you will need to have a wellness exam too.

    *Choices offers rapid HIV testing as a routine test for all of our patients in accordance with the CDC's Recommendations for HIV Testing which advise routine HIV screening of adults, adolescents, and pregnant women in health care settings in the United States. People who are infected with HIV but not aware of it are not able to take advantage of the therapies that can keep them healthy and extend their lives, nor do they have the knowledge to protect their sex or drug-use partners from becoming infected. Knowing whether one is positive or negative for HIV has great benefits in healthy decision making.
